I can bring up the Insert Field window from the command line or the button
on the upper menu that shows when a text object is being edited. I do not
however see any control to allow me to create a new field. I get the
enormous list of predefined fields but the + button that MAC OS normally
shows to add to a list object is missing.

I am relatively new to autocad but I believe that people working in the PC
environment were able to make user fields.

The workflow I am trying to set up is to create a drawing where the same
specification is referenced on multiple sheets, to avoid the error of
changing it in one place and not another I want all references to actually
be to a user variable so if I change that all references will correctly
change. A global update fields command would be helpful too, it seems to
be necessary to use the update field command on each layout.

BTW while editing a text object the only way to issue the insert field
command is from the context menu that appears at the top. The command line
and the insert field button on the annotate toolbar do not work. I assume
this is because I am in the middle of the Mtext command and that would be
issuing another command?
