Just to get the most obvious of things out of the way first - you have Excel installed, right?

 

This could simply be permissions related, as permissions issues are the most common issues regarding add-ins. If that is the case, then closing FeatureCAM, right-clicking it, and selecting Run as Administrator should straighten it out. It may also be worth 'browsing' directly to the default template again.

 

Let me know if none of that works.
