Hi,
First of all, I think the problem with the notch is simply because of your contour selection that uses the inside curve of the undercut so it cuts all of that material that should not. As far as the radius you are talking about, those seems like your ballnose either started too deep or your top facing toolpath left too much material. Could simply be a matter of wrong tool offset on the machine. I also noticed your are only machining about half of the radius all around the model which explains why you also see a concave radius at the bottom of your fillet. Your toolpath is not going deep enough. You should try adding an additional Offset to your boundary. I like to add the tool radius - Tolerance so in your case, 2.9mm should do the trick.
