As far as you Revit version, I am not sure if you may have left it off on accident but there might be another update waiting for you.

As far as your Fabrication Build, here is a link to help you determine whether you are on the latest version or not.

My experts and I did some initial testing and this is what we were able to come up with.

 

It is possible that the duplicated service lost some associate with your button mapping.  However this will not happen if you simply did a copy of the service, and changed nothing.  After reading through your initial forum post and your reply, it sounds like you are duplicating your service and possibly making changes.  

 

Specifically you mention "I've been populating a folder structure to be a "company centric" folder structure".  If you add items or (populate) to a service, you need to add a button code, if you do not add a button code then you will have an empty placeholder as Revit doesn't know what elbow to pull and populate per the convert to Fab request.  By default when you add items to a service it does not have a button code.

Can you confirm that there are button codes for the items you are adding?

 

Another possibility is that the button mapping for bends have been un-selected, or unchecked, is this something you are seeing?

In Revit there might have been a change that is also causing this if all of your Fabrication CADmep content is set up correctly.  You can manually choose what buttons you would like Revit to populate with by selecting the button to left of settings in MEP Fabrication Parts window.

Here you can choose whether you want Revit to include or ignore the button depending on your preference.
