You haven't provided a lot of information to go on.

What database configuration are you using?  Autodesk out of the box stuff?  What service are you drawing with?  What material and fittings are in the service? 

What happens if you right click on the flanges in the service and set the to "Exclude from fill", do the proper fittings come in?
