How can i do that a patient stay in a exam table area for a long time without using a staff, and also that the staff attend several patients

The first part is easy, just use a Process activity in the patient track with no staff requirements. The patient will then stay there as long as you specify without using a staff.

For the second part, you'd want to use a global process to let the staff member decide who they want to help. You might find the Patient Rounds option in the Activity Started Trigger to be useful.
