Item flying away from conveyor

Hello, finally I am measuring output and realized that in time 20391 the item want to disappear and fly out of conveyor.

It has usual label as other items - TypeOfBox and Type. Based on the value it should go to queue A4, but instead it's is running away.

Is it some kind of bug, or I can't see some detail? Thank you for help

Thank you @Jason Lightfoot I just donwloaded your version and it doesen't fly away anymore. I think it was some bug.
