How do you recover a disappeared PCI Table?

The attached model contains five individual Patient Tracks each keyed to a specific ESI. Suddenly, the supporting PCI table has completely disappeared. Moreover, the "Patient Track" Tool can no longer be added to the Toolbox. So, how do I get my PCI table back and the Tool Box operating again? See model: crmhedver4.fsm

Attached is a fixed version of your model. Somehow the PatientBin object was in a weird location in the tree.

The Patient Bin normally sits in the top level of the Tools folder:

But yours was buried way down several levels inside of another object in the FlowItem Bin:

I just moved it back out and everything works again. I don't know how it could have possible gotten there though. Let me know if you have any idea how that happened.
