Eligibility verification issue: First name doesn't match last name (?!?)

I am employed at a state college supporting faculty who use Fusion and other Autodesk products and want to learn more about it. I have twice uploaded my school photo ID as part of the verification process for a Fusion educator license, and twice I've gotten this ridiculous email: "The first name in the document does not match the last name on the verification form." Umm, yeah -- of course the first name on my ID doesn't match the last name on the verification form. But my full name on both is the same. Three strikes and I'm out, apparently, so rather than try a third time, I thought I'd better ask for help here.
