Text attributes in dynamic block not working

Hello,

I have been trying for weeks now to get this text attribute to work with with my dynamic title block but it seems that autocad civil 3d 2012 sp 4 is have lots of problems. I would appreciate if someone can fix this for me.

Here is what I would like on the right side I have what I call 6 frames

Frame 4:

In between the word date and by I would like Description centered. The word Description will not be modified.

First line above the word date 01-03-15   Issued For Bid     above the word By     AD   The word issued for bid and the A in the triangle will not change the rest will be able to modify by people.

Next line above the A in the triangle and remaining line above will be no. as 1,2, 3 etc. in the triangle with the date and some description and by the persons initial. This information will be available to edit by any individual.

Frame 5:

Street address, Title, City, State, Zip code, Telephone Number All information will be able to edit by individuals.

Frame 6:

Under the word title I would like the word COVER SHEET This word will be able to edit by any individual

The remaining info will be able to edit by individuals except the word scale an sheets

In the drawing area under issued for bid I would like a automatic diesel with the format 01/13/15
