The block does not start working until both lookups have been used for selection. Also 'custom' has been added to both lookup lists. You can avoid both of those issues by making sure that the as drawn input parameters match a row on the double lookup table and that row matches the default visibility state (topmost in the visibility dialog). I'm not really a fan of the gap between the leader start points and the callout. Otherwise, I think it's fine.
