Display Lookup Table Hangs up/freezes when opened

When editing a dynamic block using a lookup table AutoCAD freezes/hangs when trying to edit a lookup table.  When I click on the lookup table icon and select "display lookup table"  AutoCAD freezes and I need to end the AutoCAD task and restart.  This happens with existing blocks that used to work and with brand new blocks with the lookup table parameter/action.  I am using AutoCAD 2015 SP2.  This seems like a program setting problem and not an individual file/block problem.

I found the problem.  I using dual screens and the dialog box was off the right side of the screen.  I used Alt+Space to move the dialog box back to the left so I could see it.
