Block reference moving with visibility state

I have a block for manholes with various diameters. I would like the attributes to move with the visibility state chosen so that the manhole reference is close to the chosen diameter. (Diameters range from 450mm - 2400mm).

If you are just showing different sized circles, then I would not recommend visibility states for that.  Instead, use a single visibility state and within it have a circle scaled by a linear parameter.  Use a lookup for selecting the linear parameter length.  Move the attributes with the same linear parameter.
