I've noticed recently that the dimension tool seems to be canceled when you press enter to accept a size so you need to keep pressing D to add several dimensions.
I've attached a screencast, one thing to note is the dimension icon on the toolbar still shows it's active but the mouse pointer doesn't have the dimension icon.
Just noticed you can keep adding dimensions even though the mouse pointer doesn't show the dimension icon.
