Subassembly won't attach to assembly baseline

Something happened to my drawing and I can no longer create new assemblies.  I choose assembly, place the baseline down, and then go select a subassembly to add.  It simply does not add, no errors, no nothing, it just won't attach the way it always has before. 

I thought this meant my drawing had gotten corrupted, so I started a new drawing and was moving things over one by one to avoid recreating them.  Well, lo and behold I must have moved something infected because my new drawing now has the same problem.  It is looking like I may have to start over for real.  

Any Ideas what causes this to occur or how to correct?  I have attached file for your review.  Thanks!

One additional observation:  The pavement layers (not autocad layers) but that are grey in color on the pre-built assemblies (that come with autocad) are missing as well in drawings that exhibit this behavior.  The little yellow flags remain, but the layers of pavement are no longer there.  This occurred at the same time that the other behavior began.

I am sorry that I cannot be of much help on your current problem.  However, one thing that I have done for the last several years is this:

Before any save I issue an "AA" (my short name for Audit command) followed by a "QQ" (my short name for QSave).

What I am saying, I do an Audit before a Save.  I believe I catch things early.

The reason I bring this up is many times on this forum, someone is asked a question about doing an Audit and they are unfamiliar with this command.

Hi @Anonymous   Select the assembly and on the properties palette change the code set style. I changed it to all codes it the picture below.

I have been working on creation of styles for corridors, and the style I created seems to be the issue.
