Hi!
Civil 3D labels scale themselves so that they always print the same size on the paper. It doesn't matter if my viewport is at 1"=100', or 1"=1000', the labels will always adjust to have the dimensions/text height you specify in the label style. So if some label text has a height set to 0.1", you better believe that thing will print at 0.1". See Civil 3D documentation about the subject here.
It's a little confusing because this is different from the vanilla AutoCAD annotative objects (dimensions, MText, Mleaders, etc), where you have control over which scales they will appear at.

Yes, the scale in model space and paper space are separate. You can have multiple viewport/layouts with different scales, and the labels will scale themselves differently for each viewport, so that they always have the same size on paper.
