When and why do we insert intermediate grade break points while creating feature lines?

Typically, this is used when you want to drop a feature line over a surface and grab the surface elevation from the triangles. An excellent example of this is the outer limits of your property line. You can create a feature line at that location to grab the existing conditions and then those feature line are used in your proposed surface.

In general you add PI points from your side and you add elevation from your choice or from surface.
In between PI its possible, due to surface triangulation, to have other elevations that can affect your slope grade of your feature line.
So if you want to the surface (if any) to affect your feature line, then yes you have insert intermediate grade break points.
The insert intermediate grade break points are shown as circles and the PIs as triangles

Another great example of using this feature is curb islands in parking lots. You can create a feature line that drapes over your proposed surface parking area and then add that feature line to an overall proposed surface. Set that feature line relative to the surface. That way, if the parking lot surface changes, the feature line updates with it.
