Coridor transition mixing assemblies issue

Hello,
I have strange problem lately starting to happening..
On the photo bellow, i have corridor made of 4 lanes, two on each side.
When I apply cooridor transition-default slope and chose outside left lane, sometimes i get result like I applied transition to inner left, not outer left assembly. But sometimes , for the same assembly it works all well. Has anyone ever had any similar issue?

I've had a similar issue before in the past, but it's hard to diagnose with the limited information in the screenshot. If you are trying to use a changing slope on the pavement, why not use super elevation? The transition tool you are using is really good for different pavement sections, but I'd be interested to learn if it's good for slopes as well!
