Coordinate system transformation not working right

I have a consistent problem when transforming from Irish National Grid coordinate system to Irish Transverse Mercator. (I'm using ING-NG and IRENET95.IrishTM)

The transformed objects are offset approximately 40m south and 40m east of where they should be:

(magenta lines are correct location in ITM, grey lines are transformed from ING)

I haven't changed from any of the default settings in the coordinate transformation commands.

Working with your drawings >>>Here<<<, I have been unable to replicate the problem. I've tried ATTACH/QUERY both ways, Copy/Paste to Original Coordinates, WBLOCK, creating shapefiles for both ING-NG (EPSG:29902) and IRENET95.IrishTM (EPSG:2157) and MAPIMPORT'ing both. I just don't see any shift.
