Hi @kjmi,
Yup, that's wierd all right. Did you try importing with Raster Tools or using MAPCONNECT?
If those fail, please post your image or a link to it.

As Mr @Pointdump  has suggested, try MAPCONNECT.

Make sure the Map Task Pane is turned on and visible before connecting to yur image, otherwise the image may not load.

@kjmi,
Before you re-install Civil 3D, try turning off Graphic Acceleration.

Would it be possible to upload another screenshot?

This time take a screenshot of the Data Connect palette with the Add to Map button and the native coordinate system of the image showing. You may have to use the slider to reveal the image's coord sys. Like this:

It could be a coordinate system issue. The image's native coordinate system and your modelspace coordinate syste are shown in your most recent uploaded image (post 8).

Since your co-worker has success with this image then we know the image is fine. The next step is to compare his data connect palette with your data connect palette.

Can you attach this image or provide a link to it?
What version of AutoCAD do you use?
What are the units in the dwg?
What are the units of the image?
