Block in Marker Style Circle Breaking

I am having trouble with the custom blocks used in the marker styles for crossings shown in the profile.  The circles are breaking when scaled in the y direction.  I also tried to use an ellipse in the block instead of a circle and it still breaks, but at a different location.  Any ideas?  Please see the picture.

Could this be an effect from a linetype being applied or inherited by the block/label?

I am sorry, I should have added this information.  It has a continuous linetype with ByLayer set in all locations.  It also is on an insertion layer that has a continuous linetype.

What type of label is this?  A crossing pipe label, a profile view label, or ?
