All devices coming in at wrong insertion point.

When a user inserts a device from the  Tool Palette, it tries to insert at the upper left extreme of the contents of the block despite where the insertion point has been defined. For example, if the device is a box, it inserts at the upper left corner.  If it's a circle, it inserts at the intersection of the two tangents of the top quadrant and left quadrant.  The insertion point of most of the devices should be the center.  If it's wall mounted, it's should be intersection of the T leg that comes off the device.  This happens with any device the user inserts, and in any drawing, including a New drawing.  No one else is affected when inserting from this shared library.  Rebooting does not solve the issue.

Basic AutoCAD 101 rule if the problem is not the source "device": all content already in the DWG file with the same exact name is never-ever replaced by what you just drag-n-drop from a ToolPalette.

I suspect that's what you are facing in only one or a few specific DWG files, but not all files and not new files. Correct?
Or this user's TPs are accessing a library none of you are using, and it's all wrong.

It is in every drawing the user opens, and nobody else.  I've already tried reloading the company profile to no success, so i'm going to attempt a reinstall tomorrow.

Have you had a chance to investigate if the user is using the same exact library as the other users?
A re-image of another PC here vs just a reinstall may be needed (perhaps even a new log-in).

We only have on location for our library. So he's definitely using the correct one. 😉

So, i guess i didn't reload the company standard profile like i had thought.  That fixed the issue.  Still don't know what caused it behave like that.
