Layout scale incorrect in PDF

I'm at a complete loss here and I have researched numerous solutions and none have worked.

My model space is 1:1. I have scaled everything in the drawing to a known dimension on the drawing to make sure that it is 1:1.

In my layout, I scale my drawing accordingly:

This is the page setup for my layout:

Yet when I export the layout to PDF, and I change the PDF scale to 1" = 16", and measure the known dimension, I get 4.51", not 6ft.

Any idea what could be happening here? Why is the PDF scale so wrong?

And how are you measuring the dimension from the PDF exactly?
Can you also post your PDF as well.

I unfortunately cannot post the PDF due to the sensitive nature of the material. I am using Acrobat's built in measuring tool, which you can change the scale on. I have used it on other drawings we have received with correct scales and it works.

And you are confident this measuring tool is not the problem?
If you print it to paper, and use a scale, is it still wrong?

If yes, Draw a 6'x6' square in a new file.
PLOT with the same settings you did in this secret file.

Still have a problem? Post this source DWG file the PDF from it to show us: remember to ensure your plot parameters are saved in the DWG file before posting it.

If there is no problem, INSERT that secret file into this test file, explode it, and repeat the PLOT and test again.

And plot from modelspace, only for a test.

Still a problem? If not, please measure the length in paperspace again WITHOUT osnaps.

I figured it out. In the page setup, set the plot scale to 1:1, set the plot area to "Layout". Then adjust the title block/border to fit the new piece of paper, adjust the model scale to what is desired, in my case 1/16" = 1'0". Printed to PDF, adjusted the scale in PDF to 1/16" = 1'-0", and it measured everything as expected.
