Wrong Xref Relative Path Location

I have Autocad 2016
I have had this error randomly for 3+ years now on 3+ different versions of Autocad and it is still not fixed. Randomly when attaching an Xref or a Jpg with a relative location, Autocad will give it the wrong relative path. I could be inserting 6 jpg's, the first one is ok but the other 5 have the wrong address.
It can happen on any type of xref but I generally only insert jpg's and some dwg's.
Here is what the error does
First jpg path is correct
./my image 01.jpg
The bad relative paths look like this
./main folder/sub folder/sub folder/sub folder/sub folder/my image 02.jpg
It will just remove the drive letter only as the relative path. It will do this randomly. I can't find the common thread as to what makes it do it. It will do it working off of a server or locally. It has done it with short paths and long paths. It might not do it for a week. I generally just open a dwg that I worked on yesterday and most of my xrefs are missing from the drawing. This is causing issues when other people try to open drawings from 2 years ago and everything is missing. I am the only CAD guy here so no one else would know how to fix it. This has been going on for many years.
The one thing I do differently from a lot of people is I use ## in front and after the name of a folder ( ## My Folder ## ). I do this for layers in autocad as well. This makes sure the folders or layers are always at the top of the list and I don't have to go searching for it. It saves a great deal of time.
I know autocad has had issues with special characters in the past. I have a couple of Lisp routines that won't work on layers that have it. I don't know that the ## thing is the issue since it happens randomly.
