When coping and pasting, text, element and xrefs are automatically mirroring and rotating. Why?

When coping and pasting from one document to another, text, elements and xref are automatically mirroring and rotating. 

My mirrortext in set to 0. 

Can anyone help?

Rotated aka different UCS settings between the two DWG files most likely.

Is there something else I can do? The ucs settings didn't fix the issue. I have add this xref into other sheets and I am not having this issue.

Can you post both the source DWG file and the destination DWG file (with the pasted text as shown in your screenshot) here for us to look into?

In my case when this happened to me the PURGE command solved the problem and it appeared probably because you have the same element block, with the same name in the drawing you are copying to and before paste and PURGE you mast delete old name block or rename new block.
