Using Autocad to specify straightness and circularity

I am trying to specify straightness and circularity in AutoCAD. I already have the provided numbers.

Please help me out. I'm using it on a mac.

Now you reduce the description`(that even was not enough to get your work done by yourself) and ask us to help you, how should that work?
