SSM View Title and Callout Blocks not working

Hi, I have been assigned the task of researching SSM and setting up templates for our team to use as the new standards. I have read through and followed the entire Mastering Sheet Set Manager article series however I cannot get the View Title or Callout Blocks to work.
For the View Title, I have created a .dwt that has a three attributes all set to SheetSetPlaceholder. There is a View Number field, View Title field, and Viewport Scale field. In the Sheet Set Properties I have linked that .dwt in the "Label block for views" section. However, when I drag across a named view from the Model Views tab in SSM, if it shows up at all, the best I can get is the Placeholder Type being displayed (i.e. "ViewTitle" not ####, or ----). None of the fields work no matter what I try.
For the Callout Blocks, I have a Section Marker, and a reference Mtext note. In the marker I have a ViewNumber and SheetNumber attribute, and in the note I just have the SheetNumber. I right-click on the view I want referenced in the Sheet Views tab, then select Place Callout Block and place the block, but none of the fields work. In each block I have placed a field before the SheetNumber that references the Project Description (this displays the letter C as we produce Civil drawings), this attribute works (only once I have exploded the block). Again when the blocks are placed all that is displayed is the Placeholder Type.
I would love to understand this problem so I can fix it. Any help will be appreciated!

Thank you @VincentSheehan, but unfortunately that doesn't solve my issue.

That is exactly what I am trying to get to work and I understand how it is all supposed to work, but it is just not working for me for some reason. The fileds he has for the view lable and callout blocks are the same fields I am using. The only difference I noticed is when I place a view with a lable or a callout block, the Enter Attribute box doesn't pop up for me like it did for him in the video.

Was there any solution to this? I am having a similar issue. When I drop in my callout block, none of the fields are getting auto-populated with the sheet view information, and so they are not linked. Or is there a good introduction to creating those links within the block? Thanks.
