Secret Fillet Settings

Is there some secret setting that could be affect my fillet tool? If you look at the picture below, the one corner which should be nice and round like the others has like a 3 face corner/point to it. The other corners with the exact same dimensions and fillets come out fine. Even if I draw a 1x1x1 cube right now and fillet the edges, I usually end up with one bad corner. I've tried with using loop and chain and still it happens. Any ideas?

The two corners shown are not the same. It looks like the radius of the edge to the right of the upper one is the same as the corner radius.

 

The edge to right of the lower one has a flat "top" to it.

 

Not sure if that is the reason why, just pointing out they are not equals and maybe because of that, a different calculation is used, or something.
