Plot to Scale in Layout paper space

Mac Computer - 

There is nothing that I can do to print to scale in Layout - setting to scale in the viewport 1/4"=1' - model tab is correct at annotate 1:1 - doesn't matter how much I try to plot to scale - this is very frustrating - can someone explain this using the 1/4"=1' scale - identify annotations from Model, Layout, Viewport and page setup. Outline the steps - please and thank you!
