Leaders Braking When Closing/Reopening Drawing

I am trying to overlay leaders on top of each other. It initially works ok, but when I save and close the drawings some of the stacks of leaders get offset by some arbitrary amount. Not all the leaders are affect for some reason.

I have a few different UCS systems active in the drawings.  However it seems random which leaders get affected.

I have tried making a new drawing and copying everything but the leaders, then creating all new leaders. The issue persisted.

I know I could get around this by just using 1 leader and a some polylines, but I am generally just curious to why this is happening in the first place.

Are you setting PLAN to CURRENT UCS for every UCS you have in place before you start adding and layering those LEADERs? And are you snapping to actual object elements (INT, END etc.) or are these all just floating starts and ends?

Yep every time I make a new UCS I would set the PLAN. The Arrow of the leaders are snapping to a node, however the other end of the leaders are just floating

Basically my process is (this is all happening in model space): 
1) Create Layout boxes in model space that correspond to each viewport in my drawing.

2) Create UCS for each layout/viewport

3) Set PLAN to UCS for layout/viewport I am working in 

4) Set up Leaders in layout/viewport I am working in

5)Set PLAN to next layout/viewport UCS

6) Insert Leaders

7) Repeat

8) After all leaders are done in all my layouts/viewports, if I then save and restart I run into the issue

I hope that is clear, I am still relatively new to CAD

2) Do you have the latest AutoCAD version updates installed?

@adam_k_puetter Thanks for the additional information; can you share your DWG file here? First, recreate the leaders to show correctly, QSAVE the file, close the program, then share it here. Then you open it while someone here opens it and let's compare results and allow them here to find out more.
