Use TCIRCLE command on a number.

You could make a block definition containing the circle and an ATTDEF, then use the block option of the MLEADER command.  During usage it's a bit of repetitive prompting because it doesn't seem to remember the parameters you used last.

 

If you were to consider third party software, DotSoft's ToolPac contains a Leader w/Bubble and Adjacent Bubble tools to speed up this process.

I figured out how to at least use the spline leader, however I would like to have two inflection points to have more control of the curve. But yes, as you mentioned the mleader function forgets the parameters after I set it. There is no way to set this as default?

 

I've never used Attribute Definition before, how would I use this to create what I want?

Edit: I tried this and it does not work on MTEXT within an mleader annotation

To get more control points for a splined leader, select the leader, and hover on the END vertex.  you'll get the option to add a new vertex as the new end location.  After that, you can hover over any of the intermediate grips, and add additional vertices where ever you like.

Create a new MLeader style, and on the content tab, select block as the Type,  One of the default source block is "CIRCLE"
