Erratic polygon extrusion

I'm an OG user with ACAD 2000. I'm creating 8 sided prismatic truncated cones. I'm extruding octagons from the small end with a negative taper. Sometimes I get my desired solid with 8 trapezoidal faces. Other times, with the exact sequence of extrude commands, I get a solid with 8 rectangular faces and 8 additional triangular faces. I can't control which I get.

How are you making the octagons?  The wrong result sounds like the octagons have had their corners Chamfered, at a tiny chamfer distance if it's not visually apparent.  When the result with triangular faces happens, check whether the Polyline "octagon" really has only 8 vertices.

 

Also, are your "8 rectangular faces" when it goes wrong really rectangular?  When I try the situation I described, between the "triangular" faces [which are really trapezoidal with a near-zero bottom width] are faces that are not rectangular, but trapezoidal, only at less taper than your intended result:
