Drawing Scales added automatically to template drawing

Hello all 

I have a dwt drawing for our structural designers with a small number of drawing scales imbedded on the template, and on 10 out of 12 installations every time you start a new drawing from that dwt the scales shown on "Template as it should.jpg" shown below are the ones available as it should be. 
The problem happens only on 2 machines that every time you start a drawing from the same dwt there are a series of scales automatically added and altered to that drawing before you start anything, as shown below on "Added to Template file.jpg".

What is going on with these 2 machines?
I have noticed that on the 2 problematic machines the Edit Drawing Scales window show a different description of the scales (note the highlight 1:5 on both attachments are 1 paper unit=5 drawing units and the wrong one shows 1 paper unit=0.005 Meters).
I would like to believe this is something related to a system variable but can't find anything on my research. How can the same dwt file behave differently on different machines? 
I would appreciate any clues from the clever people on this forum.

Impossible if you talk about command NEW or QNEW. But we see two different issues.

a) 1:5 for example is different

b) there are more scales.. and the name indicate that there was an "insert"-workstep.

 

Are you really really REALLY sure you take this one DWT file and not an edited copy?

Please OPEN the dwt file (command open, extension = dwt, select your template file),

how is it now?

Thanks for your reply CADffm.

I am talking about QNEW and am sure they are all opening the same file set as the default template file for QNEW, that is sitting on a read only network location.

If there was an -insert it is done on the background before I can start using the file, I have checked ACAD.LSP, APPLOAD and ACADDOC.LSP for any anomalies but could not find anything.

What i wrote about the "insert" was only pointet to these *_1 scales, not for the issue with different definitions (your example with 1:5).

I think we do not talk about plain Autocad, or?
Perhaps Architecture, Civil3D or another vertical product?
Please compare the versions in ABOUT dialog.

...or some automatic executed code from a plugin inserting a block (titleblock, or layout, ...)?

 

I agree with that statement:

if you save as drawing as template, that does not contain any "xxxx_1" scales .. and you later start with command _NEW with exact that dwt-file then it also should not have any of these scales.
