Generally the lines are drawn outside the dimension-lines when the text does not fit between the dimension-lines or both arrows don't fit between the lines.
Change your dimension-style to use arrows instead of circles. Then you'll see that for dimensions sometimes the arrows don't fit in between the dimension lines, so the arrows are flipped to outside ==> and so the lines to the arrow are also shown outside.

What you can do now is to manually flip these arrows by selecting the dimension to get the grips, then place your cursor on the grippoint of the arrow you want to flip, just stay a second or so on it to get the grip-context menu, now select "Flip Arrow"
