AutoCad 2012 Block Duplication

I am inserting a block for the first time, put it on my layer and place in AutoCad.  Now for some reason every block in this particular file, if I select it on screen and copy it around will generate a new block and name it with a new number sequence.  Example, I insert 03-Office Chair it inserts but puts 03-Office Chair-flat-01 then If I copy it a new block will appear in my names objects list.  03-Office Chair-flat-02, 03-Office Chair-flat-03, 03-Office Chair-flat-04 and so on.

i don't know how it's automatically with copy cammand does these weird flat blocks appeared, all what i know that blocks such (03-Office Chair-flat-01)

usually appeard when we use flatten command line. 

maybe there is some hidden commands come a round with lisp or plug-in to your system. try to reset your autocad programe.
