Add symbol to multiple mtext insertion point and different style of text automatically?

I'm not sure how to ask for my issue.
I wish to have a certain result, but I don't know how to get there.

I have a drawing with more/less 300 mtext that defines point elevation. Like this:

I want to transform the above to this:

I'm not sure how to achieve this rather quickly and automatic. Is there a way? Note the offset between text and symbol.
In the image, I just did it manually, with a simple block and rotated mtext.

Would it be possible to get a sample drawing containing the before and after state? Perhaps two drawings may be best.
