3D Cursor's Z axis crosshair is missing

If you're trying to 'reset' everything then at the commandline type

<set it to> WORLD
PLAN
<set it to> WORLD

I figured it out. I am in isoplane / isometric drafting mode. Must have clicked that status bar button unknowingly. I never use that mode. I am now going to unload it from the status bar.
