I'm trying to create an mleader with a custom "arrowhead". I can add the arrowhead to the leader with no problem, but I have to drag it so far away from the reference tag. If I move it any closer, the arrowhead disappears. Is there something I can do to alleviate this issue?

May I ask why you need this long line in your custom arrow block? That's the problem
Looks like you created the problem to solve a problem with your arrow's placement having a gap perhaps?
It appears you want this all to be annotative too, is that correct?

If I don't draw that line, there is a gap between the arrowhead and the mleader. It seems like all the arrowheads that AutoCAD provide have a stem of sorts. See below:

EDIT: Your line's length is the problem, here they all are side by side at 1:1 scale. That's it in your posted file, I'm not sure what your screenshot is from, your custom block's size is out of whack

This is "by design" - AutoCAD hides multileader arrowhead when the first leading segment is less than twice the length of the arrowhead.
But there is a workaround.
Please look at the attached DWG - try multileader inside yellow rectangle.
Does it work for you?
If yes, then idea of the workaround is to make block elements four times bigger then it should - in your case I scaled objects inside arrowhead block 4 times.
Now you need to edit multileader style and set the scale of the arrow to 1/4:
That is all.
The new multileader style in your drawing is MLDR-REFR-DETL-1 and arrowhead block for it is REFR-ARRW-1.
