quick access toolbar icons disappear

Just recently the 8 or so icons I have had on the quick access toolbar for years have disappeared. I can't seem to attribute it to any specific change but, after adding them back, they randomly disappear again. Not sure what is causing it. Using same shortcut to load program every time. Any help would be appreciated

Windows 10 64bit

Put them back, then use WSSAVE command to save your active workspace.
Repeat for every other workspaces.

If you recently got switched to an Enterprise CUI (or use a 3rd party add-on that autoloads its own profile) you've got issues only your CAD Manager (or IT) can help resolve.

the WSSAVE command does not work. The problem seems to occur when I log out of my network profile (Microsoft user account) and into a local user account. When I switch back to the network account where I use AutoCAD the customization is "usually" gone but not always. Oddly, all of my middle button menu customizations never change.

There is no fix for that occasional random issue you have: there is something else at play, most likely at the system level, that you'll need to dive into with your IT in more detail.

the "usually gone" just means that I haven't spent a ton of time investigating "when" it happens. Let's just say that it only happens when I leave AutoCAD open on the network profile and switch to the local profile. If I close AutoCAD before switching from network profile to local profile.

That's workflow is the problem most likely then, it is not common.

Autodesk Support is likely to say the same but its worth asking like this https://knowledge.autodesk.com/customer-service/account-management/users-software/support-options#:~....
