OSNAP going to points that do not exist

I've started a new drawing and brought in an architectural floor plan to trace using a thicker lineweight for furniture layouts and millwork for a client. I noticed the drawing doing some weird things when I was tracing. My OSNAP is saying there is an intersection along lines or fractions of an inch away from lines but there's nothing there. When trying to extend or trim a line it might do the opposite without having shift held down. No layers invisible, nothing - just my plans.  It's a new file but I think something got messed up along the way. I think I've experienced this in the past but have no idea how to fix it. This is so frustrating and I have no idea how to fix it. All help appreciated.

List the line and see the Z value, is it 0 ?

Open the arch drawing and try this:

 

UCS use World, then

PLAN use Current

 

There is a chance the UCS is rotated.
