It would be a good feature if we could get additional questions if a certain answer is given. 

In this way you can get additional information/documentation if necessary. Microsoft and google Forms have this option.

Came here to actually post this feature request - conditional logic currently includes suggesting a photo, issue, etc but should also allow for a 'go to' function based on the answer provided that would jump to a certain section/question, thereby bypassing questions in between.  Similarly, an 'add another' question could be viable for duplicating the current step - think crew names ahead of signatures - currently, we have to create X number of employee name/signature fields that may or may not match the crew size filling out say, a safety form that needs to be signed off on.  Presently, those fields that are not filled out show up on a completed form - just wasted space.  

 

This one seems like it should have more traction with Autodesk.

Signatures. Very good suggestion. This would make the forms more user friendly. Now we need to make many placeholders for possible people who are joining site inductions.

 

Also the go to function (kind of conditional logic) is needed. It would decrease the number of empty questions. This enhancement would make the form usage lot more user friendly.

Agree with the comments below. It would be great to have either the option to configure the Equipment/Material pages to be purposed differently OR allow a similar style of form part that allows you to either type or select users + signature + optional fields. Each org might capture different information in the fields - OSHA 10, OSHA 30, Competent person, Hardhat #, Check-in ID# etc.
