Duct fittings yes. Ducts are system families, so you're kind of restricted there. What exactly are you looking to do by creating your own ducts?
If you mean duct systems like outside air, that's fairly easy. From your system browser (or under Families in your project browser), select the duct system you want to duplicate, go to the type properties, hit duplicate, and rename and change graphic overrides as needed.
You can create new duct type for that matter and then can map your own fittings into that. For using a specific duct type into the system, you can only assign by the system classification in mechanical settings under Duct settings/Conversion/main or Branch.
