Okay so this is very frustrating and weird.

Here I am on a nice sunny morning working on my model, just simple modeling, extrusion, edge loops etc (very simple)

 

And all of a sudden that element of my model is being completely distorted almost severely decimated, some vertices get disconnected and the best part, history deleted.

 

I didn't set up any new hotkeys.

What on earth can this be?

(happened 3 times already)

can you post screenshots or share your max file? is it an editable poly? editable mesh?

Hi,

 

It's hard to guess without a clear screenshot of the model and the modify stack,

But one possibility for this is that if you have multiple modifiers, and these modifiers work on

Sub-Object selections and not just on the object level. So maybe, while you're working down the stack,

let's say in an Edit Poly modifier, Vertex level(it can be any sub-object level), you kept some vertices selected

And, in addition to that, you didn't click on the Edit Poly title to switch to the object level.
