Radial chamfer iregularities / artifacts

I use chamfering a lot and I do get this a lot - radial chamfering works really well opposed to 'tri' chamfering, but I'm seeing this a lot. Even on the cleanest edges you will sometimes get this on a corner and I can't figure out why beyond it being buggy.

 

This was a 2 segment radial chamfer that is perfectly straight without any complex compound chamfers/edges:

Does anyone else have this, and how did you fix it?

 

(please don't reply if you've never had this or are just offering basic information - sorry, I get reply alerts and check in case it's a solution - it's not helpful and doesn't contribute to the thread, thanks)
