When Rigged FBX models are exported from Blender & opened in Max, their bones do not display correctly. Why not and how to fix?

This happens to me quite a lot.  With Blender being the cheapest 3D program to use, inevitably a lot models are made there.  Unfortunately, something clearly gets lost in translation because the bones look very wrong when opened in Max.

 

I say "look" because based on what little I can discern, they actually work correctly, but they are often the wrong size, at least appear to be sticking out at wrong angles, and worst of all, don't look like they connect to each other even though they do.  Skin still bends correctly so far as I can determine.

 

It is difficult to work with rigs that look like this, if not outright impossible.  So does anyone know why such effects occur, and how to fix them?
