Mix Tag and Spot Elevation

HI everyone,

I'm trying to create a tag that displays the type and also shows spot elevation for both "Top and Bottom" within a single tag. I'm attempting to do this in a single tag so that I can use the "Tag all" tool. Has anyone done something similar or have any ideas on how to achieve this?

here is how i would like it to look. but with one click and not 4 clicks and some moving around.

or mabye another way as long as it is easy to do many times over.
