"Item" (assembly level information) and "Instance" - where does that come from, custom iProp?

If it were two part-level iProps, then I'd suggest creating a new Custom iProp to combine the two existing properties - and that would appear as a single line in a balloon. But since one item is an assembly-level property, I think you will have to accept going crazy.

The only other suggestion/question: Could your "Instance" property be the level indicator from a multi-level structured BOM? If so, your balloon only needs to show "Item" because the item number will automatically be in the format you need.

You will need to define your own balloon, based on your own sketch symbol:

Expand "Drawing Resources" and right-click Sketch Symbols > Define New Symbol:

Then make a balloon style to use it later, picking your sketch symbol:

The 'Instance' is a iproperty (you know the newish feature , instance properties) we use it trace the materials, eg. Item 1 has a qty of 5.. so each time item 1 appears we label them 1-1, 1-2, 1-3, 1-4 and so on. (The Item no. is automatic as per the parts list, the instance is manual typed in to the property)
it would be nice if it could be on one line of text . the double balloon just doesnt look good.

You just have to add enough clearance for both, and in his case it looks like he knows the number of digits that will appear. If he can link the iProperties he needs to the sketch symbol, he should be able to pull this off.
And yes, it is the BOM/Part List item number he needs, plus the instance number which might be something you can get from the BOM if you ungroup everything and flag each one. Too much to describe in one go, so I am hoping he tests and shows how far he could get with setting up the balloon first.

I have tried with a sketch symbol, but the text is always stacked. no matter what i do.

If you have an example of a symbol i could use which would work that would be great.

Aha, I see what you mean. Yeah, no matter how I set my sketch the balloon style will not respect its text box arrangement. You could log this in the Inventor Idea station (see below). It is not something possible today:
https://forums.autodesk.com/t5/inventor-ideas/idb-p/v1232
