I encountered the same problem, all the solutions didn't work because it's a trivial error.
In the original test2.dwg file your double lookup visibility has the first value
"1111
111111
111111"
instead of "1111" you can see this also by doing CTRL+A and copy paste in your notebook or left click on top left and drag bottom right and you'll see the "111111" appearing in the rows below.
Autocad could fix this display issue because if someone were to inadvertently press enter and insert content in the lines below would not be easily able to see it and understand why the block is broken.

I've been having issues with showing states correctly in a dynamic block with multiple visibility states using Lookup Actions.

The attached drawing is for a weld symbol and is supposed to turn on/off 4 things (Leader, Flag, Circle and Tail). I finally managed to get it to work for all states except for the first visibility state.

The first state should look as shown in attached image.

Apart from this issue, I found that updating Lookup Actions or Property Lookup Tables, is not reflected in the block outside of the Block Editor.

Your reference distances are too long.

To have the first visibility state show correctly all your reference distances need to be 1.

There was also something wrong with the first 111 lookup. I deleted and recreated it and the block works now. See the attached drawing

Thanks for resolving this.

- That's the first time I've read that the arbitrary distance needs to be 1.

- With the recreation of the first lookup: Yes I had noticed something similar before, as I had had issues with visibilities showing correctly in general. When I re-wrote in the Property Lookup Table, the visibility often showed correctly. Is there a setting which would refresh instead of re-writing. Why does this occur?

I don't know why it occurs, or how to fix.

With the length parameters however they always need to be a length which you are using otherwise you get a "CUSTOM" in your dropdown list as the length doesn't correspond to any known values.

Try it out. Change one of the lengths to 5 and then test the block and select the relevant dropdown.

OK thanks that explains the CUSTOM option. I didn't get it when I changed the arbitrary Distance parameters this time, but do remember seeing it when I had made changes in the Block Editor previously. In the first selection it was there, but afterwards disappeared.
