Try adding a TurnToPoly and EditPoly modifiers before the turbosmooth.

I am having an issue with Turbosmooth. As you can see by the After 2 image, the Turbosmooth moves vertices out of place resulting in a messy mesh. On the other side ( see after 4) it is completely fine even though it is the same... Does anyone know what is the cause of this? Thank you.

My initial thought would be to check whether all the vertices have been welded in the geometry giving rise to the anomalies. e.g select a vertex with the marquee tool and see if you have selected a single vertex or multiple vertices.

No, I have xview on all the time to avoid such issues. No overlapping verts.

I've never seen such an outcome without something like a ripped mesh, or non-quad mesh. Once you figure out what caused this, please let us know. Sounds like there is another way such a result can occur.
