What actions list are you referring to?  What version of Vault are you using?  Need more information about what you're trying to accomplish.

I need to be able to add a new action to this list. It is under; Tools, Administration, Vault Settings, Behaviors, Life Cycles, Name of Transition, Transitions Tab, Edit, actions.

I don't believe that is possible in Vault at this point.  I'm not sure if this has been added to the Idea Station yet either, you may want to search there and add it if it's not there yet.
